 One Stop Shop - Online Registration

All Students K-12


This registration is for the parent(s)/legal guardian(s) who have children attending Brookhaven School District for the 2019-2020 school year. Computers & personnel will be available to assist with online registration.

                                     When:  Wednesday, July 17 & Thursday, July 18

                                      Time:  10:00 am – 6:00 pm


Parent(s)/Legal Guardian(s) are required to have a valid MS Driver’s License or MS Photo ID (obtain from MDOT License Center) in addition to two (2) Proofs of Residency (one (1) each from the lists below) to register their child/children.


 Group 1 (choose only one (1) from this list)

  1. Mortgage Statement (dated within the last 45 days)
  2. Property Tax Display Form
  3. Filed Homestead Exemption Application Form
  4. Filed Property or Warranty Deed (dated during the actual school year)
  5. Apartment or Home Lease/Agreement


Group 2 (choose only one (1) from this list)

  1. Cable Statement/Bill (includes the following: Cable One, DirecTV, or U-Verse)
  2. Water Statement/Bill
  3. Solid Waste Statement/Bill
  4. Gas Statement/Bill
  5. Electric Statement/Bill  

(Bill reading or statement dates must be within 45 days of school registration)



New students must also have the following:

MS Immunization Records

Original Certified Birth Certificate

Social Security Card

If applicable also submit:

            Withdrawal Letter from former school

            Medicaid Number

            Free & Reduced Lunch Form

            Transportation Form

            Medical Information Form

            Notice of Special Needs

 7th Grade Students (Entering, Advancing or Transferring) are required to have proof of the Tdap Vaccination

The following items will be needed to complete registration by affidavit in the Brookhaven School District:

  1. Name of Affiant (the person whose home the applicant will be staying with). ID will be required of the Affiant just like it is of the child’s parent.
  2. Name of the Parent/Guardian. ID will be required to show proof.
  3. Physical address of the Affiant has to match the ID.
  4. Name of children must be provided. If there are multiple children involved, an Affidavit must be provided to each school. If the parent has multiple children at one given school, there are spaces provide on the form for each student to be enrolled at that particular school.
  5. The relationship between the Affiant and the Parent/Guardian must be given.
  6. A reason for the Parent/Guardian needing to live with the Affiant must be given.
  7. Two proofs of residency must be provided by the Affiant. The proofs must be in the name of the Affiant and within a 45-day period of the ones that are noted:

8. Correct dates must be clearly provided and the signature of both the Affiant and Parent/Guardian must be legible.
9.   Form must be notarized by a Notary Public with a stamp that is up to date.
10. Please make sure to inform the parent that the affidavits last up to 30 days and that the school must be notified if there is any change in their living arrangements (i.e. moved into their own home or if the Affiant has moved into another home with the Parent/Guardian)

*All individuals registering with affidavits will be subject to random home visits by school district personnel. Any proof that the Parent/Guardian does not reside with the Affiant at that time will forfeit the privilege of registering with the Affidavit of Residency and will be asked to withdraw any child registered under an affidavit from the Brookhaven School District.
