2023-2024 STUDENT REGISTRATION INFORMATION
Dear Parents and Guardians,

We will utilize the online registration process for returning students and hold the annual “One-Stop-Shop” event to verify residency and complete registration for both new and returning students whose parents opt not to complete registration online.

Students will not be allowed to attend school until registration is complete.

Returning Students

• Online registration begins Tuesday, June 6th.
• Log into your Active Parent account and verify your child's information.
• Upload your proof of residency documentation using the Residency Proof Documents tab.
• Upload a copy of the parent/guardian valid MS driver’s license or state ID using the Misc. Documents tab.
• Schools will process each child's application including address and residency verification. Parents will be
contacted if additional information is required.
• Parents will receive an email confirmation once registration is complete.

New Students

• Parents may call the school and schedule an appointment with the counselor to enroll their child beginning
Tuesday, June 6th.
• All information and documents listed below are required when enrolling a new student. These items should be
brought to your appointment with the counselor.

One-Stop-Shop

• Only parents who need help with or who have not completed the registration online should attend this event.
Location: Lipsey Middle School
Dates and Times:
• Wednesday, July 19th – 10AM-6PM
• Thursday, July 20th – 10AM-6PM

Student Registration After One-Stop-Shop

• Registration will continue until noon on Monday, August 7th.
• Students must be registered by this time in order to attend the first two days of school on Tuesday, August 8th
and Wednesday, August 9th.
• Student registration will resume on Thursday, August 10th.

Residency Verification

Two current proofs of residency are required for the 2023-2024 school year.

Provide one document from each group listed below.

Note: Any unofficial lease/rental agreement
(handwritten/computer generated) must be
notarized.
Cable Bill (Cable One, Direct TV, Dish, etc.)
Mortgage Documents Telephone Bill / Internet Bill

Falsification of any information or document required for residency verification or the use of the address of another person without actually residing there may result in; a) revocation of student enrollment; b) being held liable to reimburse the district for expenses incurred to educate this student; and/or c) civil action resulting from fraud, negligent misrepresentation, and negligence. (School Board Policy JBC)

7th Grade Parents: The MS Department of Health requires all 7th grade students to receive the Tdap
vaccination. Upcoming 7th grade students will be required to present an updated MS Immunization Form 121
documenting the Tdap booster before they will be permitted to complete registration and attend classes. For more information, visit the MSDH website at http://www.healthyms.com or call 1-866-HLTHY4U (1-866-459-4948).

Required Documents for New Student Registration
The following documents are required to register a student for enrollment in the Brookhaven School District:
• Certified Birth Certificate
• Social Security Card
• Immunization Records (Mississippi Immunization Form 121)
• Withdrawal Report
• Transcript from prior school (high school only)
• Medicaid Number (if applicable)
• Notice of Special Needs (if applicable)
• Valid Mississippi Driver’s license or Mississippi ID (Parent or Guardian)
Two (2) Proofs of Residency - A post office box address will not be accepted. The school may require additional documentation or verification at any time, particularly if there is a discrepancy or a reason to question a student’s listed address. See chart above.

Affidavit of Residency
There are two circumstances when a student would need an Affidavit of Residency to register for enrollment in the Brookhaven School District. 1) The student lives full time with the parent in a residence not owned or leased by the parent. 2) The student lives full time with an adult that is not the parent because of an extreme circumstance.

Any legal guardianship formed for the purpose of establishing residence for School District attendance purposes shall not be recognized by the Brookhaven School District. MS Code §37-15-31

All affidavits of residency must be approved by district administration prior to registering a student in the Brookhaven School District.

Custodial parent(s) or legal guardian(s) residing with an adult in a home not owned or rented by the custodial parent(s) or legal guardian(s) will present documents to show legal custody. The homeowner or leaseholder must verify residency through the same process outlined above and provide a notarized affidavit of residency. The custodial parent(s) must provide one current document with their name and address matching the homeowner's or leaseholder's address from Group II listed above.
